Diabetes nursing and kaitautoko mate huka support

Who can use this service?

Whānau living with Type 2 diabetes.

What you can expect

Our registered nurses come to you and provide clinical care and health advice to support you to independently manage your condition for the best hauora outcomes.

This service also offers tangata whaiora (people seeking health) support from our Kaitautoko Mate Huka who travels across Taranaki providing whānau with basic diabetes education and emotional and psychosocial support. They link tangata whaiora with clinicians, hauora experts and kaiārahi  that can assist with their hauora journey.

While our nurse and Kaitautoko Mate Huka work with the tangata whaiora, our kaiārahi is available to awhi them and their whānau.

Our registered nurse and Kaitautoko Mate Huka can support you with:

  • Managing medication, treatment and side effects
  • Learning about Diabetes and managing it
  • Symptom monitoring
  • Goal setting and lifestyle changes
  • Access to the national Diabetes Your Life Your Journey programme
  • Working closely with the other health professionals involved in your care
  • Connecting you with other support services and organisations

This is a free service

for whānau living in North Taranaki (Ngāmotu, Waitara, Mōkau and Inglewood) and Coastal Taranaki.

There is priority for Māori and high needs clients who face barriers to receiving adequate care.

Our kaiārahi can support your whānau by:

  • Connecting you with holistic hauora support
  • Advocating on your behalf
  • Providing navigation services (see our Koiora / Life Services)
